We’re excited to announce a strategic partnership with Cognizant’s TriZetto division to integrate CareSimple’s remote patient monitoring platform directly into CareAdvance, one of the most widely adopted care management platforms among U.S. health plans.
Through this partnership, health plans can launch and scale RPM programs to improve outcomes for their members without having to build or manage new operational infrastructure.
The Problem We’re Solving

With this integration, CareSimple operates as the RPM engine inside CareAdvance. The workflow is seamless for care managers: when a nurse identifies a member who would benefit from home monitoring — blood pressure, glucose, weight, pulse oximetry — they initiate enrollment directly from CareAdvance. Everything that follows is managed by CareSimple.
We ship FDA-cleared cellular devices to the member’s home. No Wi-Fi, no apps, no setup calls. The member takes a reading, and it flows automatically into CareAdvance via native FHIR integration, appearing in the care plan where the nurse already works. If a member needs help with their device, our RN-led support team handles it.
