CMS-1848-F · Final CY 2027 rates
What does 2027 change for your RPM program?
A planning tool for the final 2027 Medicare rates: your program's revenue at 2026 and 2027 rates, side by side. Three questions give you a first estimate; your locality, other programs and plans for the year make it a full report.
What kind of organization runs the program?
This sets a starting panel size. You can change every number on the next screens.
How many patients are in your program?
Patients on the program in a typical month. Planning a new program? Enter the size you are aiming for; after you unlock, you can plan the ramp-up from zero.
Patients finish monitoring, move or stop sending readings. About 12 months is typical in Medicare claims.
How do patients engage with the program?
The defaults are where most programs land. Move them if you know your own numbers.
Remote patient monitoring (RPM) only · care management time comes on step 6
These patients bill the device and readings code (99454). We assume 10% send readings on fewer than 2 days and bill no device code that month, which leaves 20% at 2 to 15 days (99445, same rate).
Clinical staff time reviewing the readings and talking with the patient about them. It is RPM time, separate from any care management (CCM, PCM) time.
Adjust the finer shares
Each further 20 minutes bills 99458. In 2024 Medicare claims, the typical practice billed about half a 99458 for every 99457.
A second 99458. Never more than the 40-minute share.
A shorter tier (99470) for patients who need less time in a month. A patient bills this or the 20-minute code, so the two shares add up to at most 100%.
They bill no device code that month.
In 2027 your program would earn about $0

Where does your practice bill Medicare?
Medicare adjusts every rate for local costs in 109 localities, set by where the service is billed. Type your practice's ZIP code, or search by city, state, Medicare Administrative Contractor (MAC) or locality code.

Besides remote monitoring, do you bill any of these programs?
Tick each program you run, then enter how many patients it bills in a typical month. RPM treatment management (99457, 99458, 99470) is already counted from step 3: leave it out here. Care-management patients may or may not also be on remote monitoring.
Patients leaving a hospital, skilled nursing or other facility stay. TCM pays once for the 30 days after discharge, with contact within 2 business days and a face-to-face visit.
High complexity needs the visit within 7 days and high-complexity decision making. The rest bill 99495 (visit within 14 days). Use your own mix if you know it.
Count each patient once. For one patient in one month, the same practitioner can't bill APCM with CCM or PCM, or CCM with PCM, and no one bills RPM and RTM together. RPM or RTM can be billed alongside any care-management program when no time is counted twice. TCM can be billed in the same period as CCM, PCM and RPM, again without counting time twice.
Who does the monitoring work today?
January 1, 2029
From January 1, 2029, Medicare pays for RPM and RTM services only when clinical staff employed by the billing practice furnish them. Which codes this covers is pending the rule text. Until December 31, 2028, contracted staff qualify.
This answer does not change any figure.
How will your remote monitoring panel change in 2027?
So far your panel holds at 200 patients, with new patients replacing those who leave.
0 for a program that starts in January.
These are the only patients who join. About 8% of the panel leaves each month and is not replaced on top of them.
In 2027 your program would earn about $0

CMS is moving the device codes (99454, 99445) and some others to new practice-expense values in steps. Without the phase-in shows what they would pay at 2027 rates if the move were complete: a reference point CMS publishes, not a 2028 or 2029 rate. The next step for the device codes could come as early as 2028 unless the inputs change; the schedule is pending the rule text.

How we calculate
The rate for each code
Medicare prices a code by its relative value units (RVUs) for physician work, practice expense and malpractice. Each is multiplied by your locality's geographic index (GPCI), and the sum by the conversion factor: $33.4009 in 2026 and $32.6286 in 2027. Rates are non-facility (a practice or home setting).
Device codes
By law, a drop of 20% or more in a code's total RVUs is phased in over two years, and CMS limits the first year's drop to 19%. 99454 and 99445 go from 1.56 to 1.26 total RVUs; with the lower conversion factor, payment falls 21.1%. 2026: $52.11; 2027: $41.11.
Without the phase-in, 99454 would be worth 0.29 total RVUs, $9.46 nationally at the 2027 conversion factor: CMS's value once the step-by-step cut is complete. It is a reference point, not a 2028 or 2029 rate.
Treatment and care management
Treatment management (99457, 99458, 99470) is within 1.5% of 2026. Care-management codes had no policy changes; their rates moved with the conversion factor and this year's practice-expense updates, 0.5% to 4.4% lower.
What each share means
- 16 or more days: patients whose device sends readings on 16+ days in a 30-day cycle (99454).
- 2 to 15 days: what is left after the 16+ day share and the patients who send readings on fewer than 2 days (99445).
- 20 minutes: patients with at least 20 minutes of care team time in the month (99457); 40 minutes adds one 99458 and 60 minutes a second.
- 10 to 19 minutes: patients with a shorter month (99470), never the same patients as 20 minutes.
- New patients: one setup code (99453) each, in their first month.
- Care management: the patients you enter for each program, every month of the year. CCM adds up to two 99439s, PCM one 99427, and APCM bills one code per patient at its level.
Remote therapeutic monitoring, add-ons and visits
RTM works like RPM: setup (98975) for new patients, a device code for 16+ days or for 2 to 15 days, and treatment management (98979, 98980, 98981). Its patients stay as long as RPM patients, and its count holds through the year. The APCM behavioral health add-ons bill on top of the APCM code: G0570 for general behavioral health integration, G0568 in the first month of psychiatric collaborative care and G0569 after, over a six-month episode. If you count initiating visits, each new RPM and RTM patient in 2027 adds one office visit at your locality's rate.
Cash and costs
Medicare pays 80% of the allowable, less 2% sequestration; the patient or a supplemental plan owes 20%, of which you collect the share you enter. The Part B deductible is left out. Staff time is the minutes each timed code requires (10, 20 or 30), plus 30 minutes of setup and education for each new patient and your minutes per APCM patient, plus your share of unbilled time. Hours divided by patient-work hours per person give full-time staff; times their cost gives staff cost. Physician time, rent and billing costs are not included.
Your patients through the year
Each month, a share of patients leaves (one divided by the average months on the program) and new patients join. Keeping the panel steady means new patients replace those who leave, and the year is twelve times a steady month. With a set number of new patients each month, the panel changes month by month: last month's panel, less those who leave, plus the new patients. With a target, we work out the same number of new patients each month that takes your panel from its starting size to your target by the month you pick, then hold it there. The per-patient-month figure is remote-monitoring revenue divided by the RPM patient-months in the year.
